2012-13 Kindergarten Forms - including School Specific Forms
Families are now able to enter information directly on the enrollment forms and the household information form. These are fillable PDF file forms that will self-populate duplicated information. Click the “highlight fields” box on the top-right of the PDF file to help you see what lines need to be entered. Once you have entered your Household Information and student information on the forms, print the pages on single pages (no back-to-back printouts) and take them to your resident school during the early enrollment window to enroll your children. Please bring your proof of residency, children's original birth certificate or passport, immunization records and any other important information related to your student directly to your resident school. (Original documents needed, no copies please)
